Clinical Co-Ordinator (pro Rata up to £40k 30 hours)

First Achieve are currently recruiting for a Clinical Co-ordinator initially on a part time basis of 30 hours a week. Role may be full-time.

Responsible for care packages as directed by the operations manager, the co-ordinator will be responsible for day to day leadership and management of the service.

Ideally the successful candidate will have complex care nursing experience, NMC registered with current PIN and also hold either a RN adult or RN Learning Disabilities qualification.

Typical duties involve:

Care planning and reviews
Package and risk assessment
Communication with family and team meetings
Staff training and support
Appraisal and Clinical assessments
Medication ManagementSkills required are:

ITU / HDU experience
Knowledge of long term ventilation
Experience of complex care
