25hrs per week - £19047.62

Role Summary:

The post holder will have an interest and understanding of physical health and care and take responsibility for the planning, delivery and evaluation of care to a designated group of patients detained under the Mental Health Act.

To maintain a current registration with the Nursing & Midwifery Council (NMC).

The post holder will be expected to act and practice always within the framework of the Nursing and Midwifery Council Code of Practice and its supplementary guidance and in line with company policy.

To comply with NMC requirements for revalidation every three years: this will require, reflective discussions, written self-reflections on practice, and obtaining confirming evidence of continued adherence to the NMC codes of practice.

While responsibility for revalidation rests with the individual practitioner, the service will support him/her to achieve this as far as possible.

The post holder will work to maximise the effective use of resources and to deliver treatment and care in an environment that is conducive to healing and recovery.

The post holder will be expected to undertake the following duties and roles:

Identify patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.

Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

Promote patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.

Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

Resolve patient problems and needs by utilizing multidisciplinary team strategies.

Maintain safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.

Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.

Document patient care services by charting in patient and department records.

Maintain continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.

Ensure operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.

Maintain nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.

Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

Maintain a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.

Contribute to team effort by accomplishing related results as needed.

Writing care plans

Implementing plans through tasks such as preparing patients for operations

Carry out wound treatment and monitoring pulse, blood pressure and temperature

Observing and recording the condition of patients

Checking and administering drugs and injections

Assisting with tests and evaluations

Carrying out routine investigations

Responding quickly to emergencies

Patient Care:

1. Plan, deliver and evaluate the care of patients, working within the frameworks of professional governing bodies, statutory requirements and the expectations of regulatory authorities.

2. Practice within the framework of the policies and procedures of Milestones, ensuring that the planning and delivery of care is patient centred and that work is undertaken with patients in a spirit of openness and collaboration.

3. Ensure that there is consistency and cohesion in the delivery of care to each patient and that work is undertaken in accordance with the agreed plan of treatment and care.

4. Ensure that all plans of care and subsequent changes and evaluations are communicated to the wider multi-disciplinary team, ensuring continuity of approach.

5. Ensure that all plans of care, progress notes, and other elements of the Health Record are completed in a cogent, comprehensive and intelligent manner.

6. All reports for the purposes of CPA, First Tier Tribunals, Mental Health Act Managers Meetings, Monthly Reports and Section 117 reviews are completed to a standard expected by Milestones and in a timely manner.

7. Attend, where necessary, First Tier Tribunals and Mental Health Act Managers Meetings.

Staff Supervision and Support:

1. Ensure that all clinical and ancillary staff working on the shift are informed as to the work required and the specifics of delivery of care to each patient.

2. Ensure that all clinical and ancillary staff are comfortable and competent with the work that they are asked to undertake, offering support, instruction and supervision where necessary.

3. Act as a point of reference, contact and leadership for clinical and ancillary staff working within the designated care team, ensuring that there is a shared understanding of the process and focus of treatment and care for each patient.

4. To work in collaboration with the Hospital Manager and Clinical Team Leaders to performance manage identified individuals.

5. Act always in such a way as to promote an environment conducive to learning and supporting personal and professional development.

Personal Development:

1. Undertake to further skills, knowledge base and awareness in respect of issues relating to holistic healthcare practice along with meeting the aspirations of the organisation in terms of enhancing patient care

2. Engage in line management supervision to support continued personal and professional development.

3. Facilitate the learning and development of clinical and ancillary staff.

4. Participate in the process of appraisal with a view to identifying strengths and areas for development, formulating a personal professional development plan.

5. Participate in the process of clinical supervision as a means of reflection on practice with a view to enhancing personal and professional development.

Organisational:

1. To be actively involved in and support the process of clinical audit undertaking such tasks as requested.

2. To be aware of the clinical governance programme and contribute as requested.

3. Have a good working knowledge of KLOEs and ability to implement the same.

4. Participate in the Health and Safety monitoring system as required

5. Participate in the gathering and collation of data relevant to the organisation as required.

6. Participate in the internal teaching programme for clinical and ancillary staff as required.

7. Attend mandatory staff and organisational meetings.

8. Promptly address, in line with hospital policy, and report any activities, behaviour, incidences that compromise the health, wellbeing and safety of individuals, team and /or the organisation.

9. Undertake any other reasonable duty as requested by Line Manager

Qualifications

Diploma/Degree in Nursing

RGN/RMN or RNLD

Essential Knowledge, Understanding and Values
